First Visit
Prior to your first visit, we need to have you complete your new patient forms, which are available to complete here on our website at your convenience. They are located under "Forms" on the navigation bar to the left, after completing the forms and clicking "Submit", they will be sent to our office computers. If you are unable to complete these forms on-line, please arrive at our office 15 minutes prior to your appointment so we can get your appointment started on time. Your first visit to Citrus Park Dental will be a full introduction to our office, including a chance to meet our team and Dr. Muir. You will receive a full dental exam, x-rays, review of your medical history, and insurance verification, if applicable. On your first visit please plan to spend one and a half to two hours with us as we get to know each other. Once you have had your x-rays and comprehensive exam from Dr. Muir, you will be given a written estimate of dental treatment and the fees associated with each procedure. If time permits, you have the option of starting your treatment with Dr. Muir or our Hygiene department at the first visit. We look forward to meeting you.
